New Patients
Welcome and Congratulations on taking the first step to feeling better and getting back on track to your health!
I make sure all patients feel welcome, safe, and cared for as an individual.
I like to schedule a Meet and Greet with every potentially new patient. This is a free-15 minute consult for us to get to know one another and make sure we are a good fit prior to scheduling a 2 hour new patient visit. If we both decide that we in fact have similar ideas about your health, we can schedule you for your first visit. You will then receive an email invitation to the patient portal at that time with a New Patient Questionnaire, PNM Financial Agreement, and Informed Consent/HIPAA forms. These forms are a bit lengthy and I respect your time. Please take the time to answer the questions with some thought. Healing is a journey and this is your first step with me. I ask that you please submit this information through the electronic portal at least 24 hours prior to your first Naturopathic visit. This gives me time to review your information before you arrive in order to make the most of our one-on-one time together. If you have lab work or imaging within the last 6 months, please bring a copy with you or upload it to your patient portal.
Payments:
All payments are due at the time of service. Current accepted methods of payment are preferred cash or check. I also take HSA cards and all credit cards. If you need to make other arrangements, please talk to me directly prior to your appointment.
I do not accept insurance plans, but am happy to create a Superbill that you can submit to your insurance company yourself for possible reimbursement depending on your insurance plan.
Cancellation Policy:
For cancellation or rescheduling, please understand that as much notice as possible is appreciated. Please note, however, that a minimum notice of 24 hours is required. If you fail to give the full 24 hour notice, a $50.00 fee will be charged to your account.
If you are unable to keep an appointment, and have not notified us, you will be charged for the entire appointment. Thank you for your understanding.
